As a Nurse Practitioner you would support individuals with no fixed address through the provision of advanced health care services. Care is provided through partnerships with different AHS teams, the Ministry of Community and Social Services, municipalities, and community organizations. Your duties will include prescribing, managing opioid agonist therapy and withdrawal management, assessing clients with chronic and persistent mental illness/substance use and physical health concerns, providing therapeutic interventions, advocating and supporting clients with psychosocial supports.

In addition to a competitive rate of pay, AHS currently has a Remote Retention Allowance in place to an annual maximum of $3,000. This allowance is non-pensionable and is payable on an hourly basis for all hours paid at the basic rate of pay.
Description:
Reporting to the Manager, Complex Needs, Addiction and Mental Health, the Nurse Practitioner (NP) is a member of an interdisciplinary health and treatment team supporting individuals experiencing homelessness in Grande Prairie.
The position provides care for clients who are socially vulnerable and require support for addiction and mental health treatment as well as medical supports. Assessment, medication recommendations and prescribing, follow ups and collaborations with nursing and allied professionals will be critical functions of this position. The NP will provide leadership and support to any medication and complex care support for clients and teams in Grande Prairie supporting individuals with no fixed address or precarious housing situations, including the outreach services component of the team, in collaboration with community addiction and mental health service providers, nursing staff, community paramedics, and local area staff and managers. The NP will liaise with various agencies, community services providers including physicians and other NPs, and various pharmacies.
The Nurse Practitioner is an advanced practice registered nurse with knowledge and skills in health assessment, diagnostic reasoning, and health management decision-making at the individual, family and community client levels. The NP has developed advanced competencies in the areas of health promotion, injury and illness prevention, family health, community development and planning, health assessment and diagnosis, health care management and role responsibility. The nurse practitioner advocates for clients and supports the integration of harm reduction into practice.
Requirements
Required Qualifications:
Successful completion of an approved Nurse Practitioner Program or equivalent qualifications or approval. Eligible for licensure with CARNA as a Nurse Practitioner (NP) in the Family/All Ages Stream. Experience in a wide range of assessment, treatment and therapeutic interventions, health promotion, diagnostic procedures, and follow-up care in the community considered an asset. A demonstrated clear pattern of professional and personal development. Must understand and practice within full scope of their practice in an interdisciplinary team environment. Demonstrated ability to manage change and create innovative solutions for complex and diverse issues. Proven ability to foster partnerships and to achieve organizational goals within an organization. Strong track record in establishing and maintaining effective working relationships with internal and external stakeholders. Effective leadership abilities combined with a strong commitment to operating within a team environment and the ability to motivate and inspire other to achieve common goals. Excellent analytical and decision-making skills. Ability to plan, organize and manage short and long term plans and projects and deliver customer-focused results.
Additional Required Qualifications:
CPR (Basic Rescuer). Possession of a valid driver’s license.
Preferred Qualifications:
Clinical Experience with addiction and mental health. Experience with working with vulnerable populations. Experience delivering services in virtual platforms. Knowledge of opioid agonist therapies. Experience with interdisciplinary teams.
