Transforming Health Care: One Pathway at a Time
The Provincial Pathways Unit (PPU) is a team within AHS Primary Health Care dedicated to the development of clinical and patient pathways using collaboration and co-design between patient and family advisors, primary care providers, and specialists. Clinical pathways designed for use in primary care, aim to support providers to care for patients within their communities and to support transitions to specialty care when required. Patient pathways are brief, easy to read patient handouts that mirror the information in their corresponding clinical pathways. Clinical and Patient pathways are housed on Alberta’s Pathway Hub. The Hub serves as an easy-to-use, central location for trusted, evidence-informed clinical, patient and referral pathways.
