Island Health has exciting opportunities for Nurse Practitioners to work in the Victoria area at new Urgent & Primary Care Centres (UPCCs). Join a dedicated team of primary care providers and other allied health staff to collectively deliver integrated team-based care to patients.
Urgent & Primary Care Centres (UPCCs) are a core component of comprehensive primary care delivery in a local primary care network (PCN). They are a highly flexible and team-based resource designed to complement existing primary care capacity and fill service gaps in the PCN in which they are situated. UPCCs provide a mix of primary care service, including urgent primary care, episodic care for users without a primary care provider, and longitudinal primary care for a subset of users who are attached to the UPCC.
Located in beautiful Victoria on Vancouver Island, BC, UPCCs are bright new clinics offering turn-key operation.
UPCCs operate through a team-based approach and can include services provided by family physicians, nurse practitioners, registered nurses, mental health and substance use consultants and social workers, as required by the local population.

RESPONSIBILITIES
Island Health looks forward to supporting primary care providers and the health care team in providing innovative primary care services that are at the forefront of primary care improvement in BC. Modeled after best practices, primary care providers will have the opportunity to optimize data from the clinic information system to improve patient care.
There is a requirement to establish a minimum panel and, in some of our clinics, to participate in urgent care. As the most responsible primary care provider, after hours care for patients is inherent in the services. The Nurse Practitioner will participate in an after-hours call group, sharing after-hour coverage with other primary care providers (Family Practitioners and Nurse Practitioners) at the clinic.

Full Nurse Practitioner licensure with the BC College of Nurses and Midwives (BCCNM) AND the successful candidates will be required to obtain Provisional Privileges at Victoria General Hospital and Royal Jubilee Hospital
