Role Overview:
As a Nurse Practitioner at Preventous, you will play a vital role in delivering patient care, with an emphasis on prevention and optimizing health. You will work collaboratively with our team of physicians, dietitians, kinesiologists, and other healthcare professionals to develop and implement comprehensive health plans for our patients. This role involves coordinating patient care, managing day-to-day health concerns, and promoting overall well-being through a preventive care model.
Day-to-Day Responsibilities:
Coordinating and delivering patient care in family medicine with a focus on prevention.
Managing patient intake, triage, and diagnosing illnesses.
Performing Total Health Assessments and Reviews for patients.
Ordering and interpreting laboratory tests.
Counseling patients on mental health and lifestyle habits for disease prevention.
Prescribing medication and creating detailed follow-up plans.
Making referrals and coordinating diagnostic tests in collaboration with family physicians.
Collaborating with dietitians and kinesiologists to optimize patient care plans.

Requirements
Qualifications:
Master of Nursing – Nurse Practitioner or post-master’s diploma.
Active registration with CARNA (College and Association of Registered Nurses of Alberta) in good standing.
CPR certification.
Experience treating diverse populations, including both adults and children.
Knowledge and experience in primary care.
Current experience in chronic disease management is preferred.
Advanced clinical nursing skills within the scope of a Family Nurse Practitioner (FNP); additional relevant training is an asset.
Experience in mental health care is an asset.
Proficiency in program planning, implementation, and evaluation.
Ability to take initiative and apply proactive/preventative care.
Exceptional organizational, interpersonal, and communication skills.
Ability to work collaboratively within a multidisciplinary team.
Competency with EMR systems and general computer proficiency.
