Description of Duties and Responsibilities

Reporting directly to the Clinical Director – Nurse Practitioner, the Nurse Practitioner is responsible for comprehensive assessments, planning, implementation, prescribing medications, and evaluation of patient care. They will perform a variety of nursing duties by administering safe, effective nursing according to clinical policies, procedures, and evidenced-based nursing care standards. The Nurse Practitioner works within a concurrent capable, recovery-oriented framework, and will oversee the LPNs and be part of the direct daily provisions.

  • Provide primary care to patients experiencing withdrawal and to patients in need of medical stabilization
  • Perform physical exams and patient observations
  • Record patient medical histories and symptoms
  • Create patient care plans and contribute to existing plans
  • Order, administer and analyze diagnostic tests.
  • Diagnose health issues
  • Prescribe necessary medications as required
  • Detect changes in a patient’s health and change treatment plans as necessary
  • Administer medicines and treatments
  • Evaluate responses to treatments and medications
  • Lead a team of LPNs
  • Ensure consistent care across rotations
  • Train patients and families on managing illnesses or injuries and disease prevention
  • Orient and motivate clients toward appropriate daily functioning, life skills, physical health mental health and substance use support services from a culturally sensitive lens
  • Develop and maintain a network of community service providers
  • Provide supported referrals to external service providers
  • Assess health, mental health, substance use and functional challenges within scope of practice
  • Research and participate in relevant training opportunities
  • Other duties or tasks as required by the manager

Case Management

  • Track and document client progress along care plans, internal databases, and case files
  • Liaison with and provide on-going support to referring agencies and client support workers regarding clinical issues
  • Work with in-house social workers to advance client care plan
  • Lead a team of LPNs to meet the immediate medical needs of clients
  • Contribute to joint client service planning and case reviews
  • Provide specialized medical expertise and service supports to detox clients
  • Assist in the development and preparation of team resources and materials

Requirements

  • Must have a Nurse Practitioners license issued by CARNA, and a minimum of 3 years’ experience working as a NP
  • Master’s degree in Nursing from an accredited nursing program
  • Must have prescription authorization
  • Ability to communicate effectively, in verbal and written form, with social and medical partners at various levels, including patients, family members, physicians, pharmacists, and representatives of the community.
  • Experience working with addictions is an asset
  • Knowledge of disease prevention
  • Knowledge of trauma-informed service provision approaches and best practices
  • Ability to work as a respectful and engaged member of a multi-disciplinary team
  • Demonstrated level of maturity, including ability to work in an independent and ethical manner respecting staff and client confidentiality
  • Must be able to provide a current Police Information and Vulnerable Record Check
