Description

As a member of the interprofessional team the Nurse Practitioner works at full scope of practice, providing comprehensive nursing services and care to patients within the Primary Care Network. The Nurse Practitioner will assess, plan, implement and evaluate patients of all ages. The Nurse Practitioner promotes an understanding of the PCN and PCN services, working with family physicians in the community, other health professionals and local and regional programming in an interprofessional environment to ensure a high quality, evidence‐based health care to the practice population.

Requirements

Education/Experience:
• Master of Nursing, Family Nurse Practitioner focus.
• Current College & Association of Registered Nurses license.
• Current Basic Life Support (BLS) for Healthcare provider.
• License to work with the family/all ages patient population.
• Minimum 3 years of related experience in a primary health care, community health nursing, or ambulatory care nursing, with an emphasis on primary care services (primary and secondary prevention, intervention, and maintenance/support). Other nursing experience will be considered.
• Knowledge of community-based resources.
• Electronic Medical Record, and other databases such at NetCare experience preferred.

• Experience in group facilitation/instruction.
• Experience in health behaviour change methods.
