Purchase of this product will grant your NP clinic a listing on our Find an NP platform for one year. By purchasing this service, you agree to allow NPAA to list your clinic contact information as part of the public directory for the purpose of assisting patients in locating and contacting Nurse Practitioners (NPs).
Please note: all listings are subject to review before being added to our system. This process typically takes up to 5 business days.

Consent Agreement
- I understand that my participation in the NPAA directory is completely voluntary, and I can choose whether or not to be included.
- I understand that clinic advertisements on the NPAA website is free for NPAA Membership PLUS members. Non-members and regular members will incur fees as outlined on the NPAA website.
- The NPAA has the right to decline any postings that do not align with our mission and values and or fall outside our advertisement policy.
- By filling out this form, I consent to the collection, use, and disclosure of my personal information as described in the Privacy Policy. I understand that only contact and practice information will be included in the directory for the purpose of helping patients locate and contact me.
- I agree to provide accurate and up-to-date information for inclusion in the NPAA directory. I will notify the NPAA of any changes to my contact information or clinic details to ensure the directory remains current.
- I understand that I have the right to opt-out of the NPAA directory at any time. Should I choose to opt-out, I will notify NPAA in writing or electronically, and my information will be removed from the directory.
- I understand that NPAA will not share my personal information. Patients will contact the clinic utilizing the information shared by the NP consenting to this posting.
